NOVEMBER: Diabetes Awareness Month

FREE Eye and Foot Exams in Illinois

The Illinois Diabetes Prevention and Control Program in partnership with the Illinois Society for the Prevention of Blindness, the Illinois Podiatric Medical Association and the American Diabetes Association urge people with diabetes to take advantage of the free services provided through the Eye Care Aware Program during the month
of November and the Foot Care Aware Program during November and December.
Volunteer ophthalmologists and podiatrists throughout Illinois will set up appointments to identify diabetic eye disease and foot complications for people diagnosed with diabetes and who have not had an eye or foot examinations in the past twelve months.

Mark your calendar now to call the IDHS at 1-800-323-4769 for the name(s) and telephone number(s) of participating ophthalmologists and podiatrists. When you call the doctor(s) for an appointment, the Eye Care Aware Program and/or Foot Care Aware Program should be mentioned.

With continuing promotion of the importance of both the Eye Care Aware Program and the Foot Care Aware Program, it is hoped that more Illinois residents will be helped in preventing needless blindness and other complications due to diabetes.
